Admissions

To apply to Chicago Makeup School, please fill out the Admissions Form below. We will then contact you for the next step of our enrollment process.

This is the form to begin the enrollment process, not to ask us questions only. Please go to our Contact Us page if you have questions before enrolling. 

Students must be 18 years old to enroll. Those under 18 (no younger than 16) need written parental consent to register or a letter from their high school.  Proof of identification and age is required.

High School reading level is required. 

Chicago Makeup School does not provide financial aid.

A NON-REFUNDABLE DEPOSIT  of $800 is required upon filling out this form. The deposit will be deducted from the tuition.
